Medical Forms
     

Class 1 (update annually for all participants)

Activity: Day camp, overnight hike, or other programs not exceeding 72 hours, with level activity similar to that of home or school.  Current personal health and medical summary is attested by parents to be accurate. 

Class 2 (update once every 36 months, and under 40 yrs. of age.)

Activity: Resident camp or any other activity such as backpacking, tour camping, or recreational sports involving events longer than 72 consecutive hours.  Current personal health and medical summary is attested by physician to be accurate. 

Class 3 (update once every 12 months, and over 40 yrs. of age.)

This includes youth and adult members participating in high-adventure activities, athletic competition, and world jamborees.  Also applies to all  Wood Badge participants/staff regardless of age.
